Use this service to enhance your summary care record.

Enhancing your summary care record would include the following information on your record:

  • Significant medical history (past and present)
  • Reason for medication
  • Anticipatory care information (important in the management of long term conditions)
  • Communication preferences
  • End of life care information
  • Immunisations

You can opt in for an enhanced summary care record at any time.

You can use this service if you:

  • are registered at the surgery

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
